HLTV was established in 2002 by Martin "Martin" Rosenbæk and Per "Nomad" Lambæk. Originally, the site was designed to host "Half-Life TV" IP addresses and demos, enabling fans to watch Counter-Strike matches. Over the years, HLTV has expanded its offerings to include news, results, statistics, and more.

The website shifted its focus from Counter-Strike 1.6 to Counter-Strike: Global Offensive in 2012, and later to Counter-Strike 2 in 2023.

The Top 20 Players of the Year ranking was introduced in 2010 and has been an annual feature since, except for 2012, which was a transitional year between game versions.

In 2015, HLTV introduced a weekly updated team world ranking, widely used by tournament organizers for invites and seeding. The site also awards HLTV MVP medals to standout players at top-tier tournaments in CS:GO and continues this tradition in CS2.

In 2020, HLTV and its regional sister site Dust2.dk were acquired by Better Collective, a Danish digital sports media group.
